“AZA–VEN vs CLAD–LDAC–VEN: no winner yet.

CLAD provides elegant ara-CTP amplification. But comparing 21 days of VEN with CLAD against only 14 days with AZA is not a clean comparison of backbones.

OPTI-AML warns us: 14-day VEN failed noninferiority to 28 days, with the greatest loss of CR in NPM1/IDH2 AML.

Shortening VEN may disproportionately reduce remission in the AML genotypes most sensitive to VEN.

Match VEN exposure, genetics, fitness and transplantation before crowning a winner.”
