“Does purulent intra – abdominal fluid at appendectomy automatically warrant postoperative antibiotics?
An interesting study in Annals of Surgery challenges this common practice. In patients undergoing appendectomy without abscess or generalized peritonitis, the presence of intra – abdominal fluid, including purulent fluid, was not independently associated with postoperative infectious complications.
Postoperative antibiotics also did not appear to reduce infectious outcomes. Importantly, these findings should not be extrapolated to true perforated appendicitis or generalized peritonitis. Taken together, these findings suggest that purulent fluid alone does not necessarily equal perforation or mandate postoperative antibiotics.”
