Our review, ‘Hold the Line: Local Control Strategies in Oligoprogressive Gynecologic Cancers During Systemic Therapy’, explores a rapidly evolving and highly relevant topic in gynecologic oncology: how local treatments may help maintain the benefit of ongoing effective systemic therapies (often in maintenance setting) in selected patients with oligoprogressive disease.
- Locoregional therapy may help control oligoprogression during otherwise effective systemic treatment, prolong NEST-FS, and preserve quality of life
- Strongest evidence supports this strategy in PARPi-treated ovarian cancer
- Data in endometrial and cervical cancers remain limited and exploratory
- Combining SBRT with targeted therapies shows promise but lacks standardization

This is such a timely and engaging topic in gynecologic oncology. Significant advances have been made in recent years – especially for ovarian cancer patients receiving PARPis – but many questions remain open.
