“New from our team OSU/James Gynecologic Oncology: does the order of mirvetuximab soravtansine (Mirv) and weekly paclitaxel matter in platinum resistant ovarian cancer?
In our retrospective cohort of 62 patients who received both agents, sequencing did not significantly affect response to either drug. But the numbers are worth a closer look. Response to Mirv was essentially identical regardless of sequence (35% when given first vs 34% when given after paclitaxel). Response to weekly paclitaxel, however, trended higher when it was given first (73%) compared to after Mirv (56%), a seventeen-point spread that did not reach statistical significance but is too large to ignore.
Our interpretation: this may reflect a true sequencing effect, a line of therapy effect, or both intertwined. Clinical benefit rate was consistently higher for whichever drug was given first, suggesting some of what we are seeing is simply the biology of earlier versus later lines of treatment.
These findings should not change practice today, but they generate a clear hypothesis for prospective, multi-institutional study. As we now have several effective options in PROC, sequencing strategy deserves the same rigor we apply to drug selection itself.
