Aurora Mirabile։ A Study That Highlights the Role of CGA in Older Patients With Locally Advanced HNSCC

Aurora Mirabile, Consultant Medical Oncologist, Head of Head and Neck Cancer Diagnosis and Treatment Section, Division of Otorhinolaryngology, IRCCS Ospedale San Raffaele, shared Paolo Bossi’s, Head of the Head and Neck Medical Oncology Unit and Associate Professor of Medical Oncology at Humanitas University and Humanitas Cancer Center, post on LinkedIn, adding:

“Our prospective multicenter study highlights the role of Comprehensive Geriatric Assessment (CGA) in older patients with locally advanced HNSCC.

In 101 patients ≥65 years:

  • CGA led to a change in treatment strategy in ~12% of cases
  • Revealed a substantial burden of unmet supportive care needs
  • Enabled a more personalized, multidimensional approach to care

Key takeaway:

CGA may not always alter oncologic decisions, but it is critical to optimize patient selection, tailor supportive care, and refine multidisciplinary management.

I am grateful to Prof. Paolo Bossi and Dr. Maria Cossu Rocca for involving me in this work.”

Quoting Paolo Bossipost:

“A prospective trial employing a comprehensive geriatric assessment (CGA) to help the MDT in defining the treatment for locally advanced head and neck cancer patients.

Main messages:

  • After CGA‑guided multidisciplinary discussion, the initial treatment choice was revised in about 12% of cases.
  • CGA results seem to mainly change MDT decisions regarding supportive care needs before curative treatment.
  • CGA could guide corrective interventions and offer more tailored treatment strategies for older HNSCC patients.

We need to increase clinical research in the older cancer populations, by adopting validated tools for frailty and defining their support to the best treatment allocation.”
