“Is one negative ctDNA result really enough?
The prospective INTERCEPT study in stage II–IV CRC raises an important point about MRD surveillance:
- Single-timepoint testing: sensitivity 48.5%
- Longitudinal testing: sensitivity 79.9%
This is more than a numerical difference.
Perhaps we should stop thinking of MRD as a single positive/negative result and start thinking of it as a dynamic trajectory over time.
MRD is a trajectory, not a snapshot.
But the key question remains: Will detecting molecular recurrence earlier – and acting on it – actually improve outcomes?
Are we ready for longitudinal MRD-guided surveillance in routine practice?”
