“New Study on PMRT and Implant-Based Breast Reconstruction
A systematic review and meta-analysis published today in Breast Cancer by Mami Ogita et al. for the 2022 Japanese Breast Cancer Society Clinical Practice Guideline highlights an important clinical concern:
Postmastectomy radiation therapy (PMRT) in implant-based breast reconstruction is associated with significantly higher risks of:
- Major complications (OR 2.62)
- Reconstruction failure (OR 2.53)
- Capsular contracture (OR 9.63)
- Poorer cosmetic outcomes (OR 3.55)
However, these results are not universally generalisable.
Outcomes are strongly influenced by:
- Surgical technique (e.g., prepectoral vs subpectoral approach)
- Use of acellular dermal matrix (ADM)
- Type of implant and patient selection
- Surgeon’s expertise
In specialized hands and with modern techniques, good results are achievable even in the setting of PMRT.
The takeaway:
While PMRT undeniably increases complication risks, careful patient selection, transparent counseling, and tailored surgical strategies remain essential.”
