“RT in Prostate Cancer: Do Relapse Patterns Differ Between Hypofractionation and Conventional RT?
Objective:
- Compare relapse patterns between HFRT and CFRT in IR PCa.
Methods:
- Post-hoc analysis of 274 (130 HFRT; 144 CFRT) from PROFIT trial.
Treatment:
- HFRT: 60 Gy/20 fr (4 w).
- CFRT: 78 Gy/39 fr (7–8 w).
No ADT allowed.
Follow-up: 6.4 y
Relapse detected by: PSMA PET-CT, choline PET-CT, MRI, bone images.
Results:
- Relapse rate: 23% (HFRT: 24.3%, CFRT: 19.4%).
Time to relapse:
- Local: 4.9y.
- Pelvic nodes: 3.96 y
- Extra-pelvic nodes: 2.95 y
- Bone: 3.6 y
Relapse patterns:
- Extraprostatic: 69% (pelvic/extra-pelvic nodes, bone)
- Intraprostatic only in 31%
- PSA, Roach score, and unfavorable Zumsteg classification predicted relapse.
Treatment at First Relapse:
- ADT: 65% (41/63)
- Salvage pelvic : 25% (16/63)
- Prostate local therapy: 11% (7/63)
