Nicole Lisek and Myint review the evolving evidence for early on-treatment ctDNA kinetics across androgen receptor pathway inhibitors (ARPIs), chemotherapy, PARP inhibitors, and PSMA-targeted radioligand therapy
The evidence is strongest in ARPI-treated mCRPC, where persistent ctDNA detection early during treatment has been associated with shorter progression-free and overall survival. Emerging data also suggest potential value across other treatment classes
Perhaps most compelling is where ctDNA may fit clinically: not as a replacement for PSA or imaging, but as a complementary biomarker when conventional measures are delayed, discordant, or biologically ambiguous
Challenges remain, including assay heterogeneity, lack of standardized response thresholds and sampling time points, and the need for prospective validation.”
