NHS-Galleri (n=142,000), the largest RCT of an MCED test:

• NEJM: primary missed. Stage III/IV (12 cancers): IRR 1.03 (0.92–1.14), P=0.63
• Nature Medicine: spec 99.5% | PPV 46–58% | episode sens ~27% in rounds 2–3

1. Very specific, but misses ~3 in 4 cancers once the prevalent pool is gone
2. Fewer stage IV, more stage III. Late-stage burden unchanged

Accurate ≠ useful. Mortality will decide.”
