“Are we close to universal blood-based cancer screening? Not yet.
NHS-Galleri (n=142,000), the largest RCT of an MCED test:
- NEJM: primary missed. Stage III/IV (12 cancers): IRR 1.03 (0.92–1.14), P=0.63
- Nature Medicine: spec 99.5%:PPV 46–58%:episode sens ~27% in rounds 2-3 – Very specific, but misses ~3 in 4 cancers once the prevalent pool is gone. Fewer stage IV, more stage III. Late-stage burden unchanged
Accurate is not useful. Mortality will decide.”
