The case reports describe two patients with initially HR1/HER2- early breast cancer who developed metastatic relapse with triple-negative transformation. In both cases, sole reliance on ctDNA would have resulted in inappropriate systemic therapy selection. This highlights the need to combine tissue and liquid biopsy in metastatic breast cancer.
