“One of the highlights of AAPM 2026 was Sunday’s lively Protons vs. Photons Debate.

Helen Shih presented the case for protons, highlighting the physical advantages of the Bragg peak, reduced dose to normal tissues, and growing evidence for reduced toxicity and improved outcomes in selected diseases including leptomeningeal disease, pediatric cancers, esophageal cancer, and head and neck cancer.

Lauren Henke, MD, MSCI made a compelling case for photons, reminding us that dosimetric superiority does not always translate into clinical benefit.

She emphasized the importance of randomized trials, cost-effectiveness, accessibility, and careful patient selection, while challenging us to critically evaluate where proton therapy truly adds value.

The technical physics perspectives from Carri Glide-Hurst and Kundan Thind added important insights into the strengths, limitations, and practical considerations of both modalities.

The biggest takeaway? There is no universal winner.

The best radiation therapy is the one that delivers the right treatment to the right patient.

As technology advances and clinical evidence grows, thoughtful patient selection will continue to guide optimal care.
