Hassan Hashmi Shares Findings from a Study of ERAS Implementation Across 86 Hospitals

Hassan Hashmi, Colorectal and General Surgeon at Las Vegas Surgical Associates, shared on X:

“Why do some hospitals improve colorectal ERAS adherence while others stall?

A study recently published in Annals of Surgery examined 86 hospitals and found that there is no single formula for implementation. Three context-specific patterns together accounted for more than 70% of hospitals with high improvement in ERAS adherence:

  • Hospitals serving lower-SES populations, estimated by Medicaid discharge volume, improved when they were ready to change, despite challenges with team skills and cohesion.
  • Lower-volume hospitals improved when readiness to change was paired with staff buy-in, meaning broad team support for using the ERAS pathway.
  • Higher-volume hospitals improved when teams had enough time for implementation activities, such as training, coordination, and tracking adherence, along with a manageable workload, even when leadership support was limited.

The takeaway: Assess local conditions and focus on factors that can be changed. The study measured improvement in ERAS adherence, not patient outcomes.”
