“473,000 Americans died of ischemic heart disease in 2023.

88.7% of those deaths were attributable to modifiable risk factors.

GBD 2023 in JAMA Cardiology, all 50 states, 1990 to 2023.

Top risk factors:

  • High systolic blood pressure: 47.2% of IHD deaths
  • Diet: 38.6%
  • High LDL cholesterol: 28.5%
  • High fasting plasma glucose: 18.0%

These risks overlap through shared biological pathways, so the percentages don’t sum to 100%. The age-standardized death rate is down 58.7% since 1990, but the pace has slowed.

Fasting glucose and BMI are the 2 risks still climbing. GLP-1 drugs act on both, so the next GBD cycle may read differently.
