“Updated ESMO Clinical Practice Guidelines deliver a refined framework for the multidisciplinary management of metastatic colorectal cancer.
Managing metastatic colorectal cancer (mCRC) requires navigating an increasingly complex landscape of molecular biomarkers, targeted therapies and evolving locoregional strategies. Standardizing evidence-based care across diverse clinical scenarios remains essential to optimize patient outcomes.
Published in Annals of Oncology on behalf of the ESMO Guidelines Committee, the updated guideline co-authored by Michel Ducreux (Gustave Roussy) establishes structured therapeutic algorithms driven by MMR/MSI status, ECOG performance status, primary tumour location and extended molecular profiling (RAS, BRAF, HER2, KRAS G12C).
Key updates include first-line FOLFOX-encorafenib-cetuximab for BRAF V600E-mutated tumours, ctDNA-guided anti-EGFR rechallenge in the refractory setting, and liver transplantation as a curative-intent option for highly selected patients with unresectable liver-limited disease.
By clarifying decision-making criteria from first-line to late-line therapy and across locoregional interventions, these recommendations provide a reference framework for clinical practice in mCRC.”
