“Bladder cancer paper in European Urology evaluating RNA biomarker for benefit from first-line EV (enfortumab vedotin) + pembrolizumab for advanced urothelial carcinoma:
In this retrospective real-world analysis, high pretreatment tumor NECTIN4 RNA expression was independently associated with longer survival (adjusted HR 0.62) and more favorable response (adjusted odds ratio 2.64) with first-line EV + pembro, but not with pembro monotherapy.
High tumor mutational burden (TMB) + high NECTIN4 RNA (double-high subgroup) exhibited particularly favorable outcomes vs the double-low group (survival HR 0.61 and best response OR 3.09), suggesting complementarity of these biomarkers.
In this context, other data from Markus Eckstein, Niklas Klümper and colleagues have shown NECTIN4 gene amplification and membranous NECTIN4 expression to be associated with better outcomes with EV monotherapy.
Thus, further study and prospective evaluation is necessary to allow the use of a combination of genomics + transcriptomics + proteomics biomarker to enhance precision oncology when using EV + pembro in the clinic.
