“I’m pleased to share that our NeoCarbo study is now in press in npj Breast Cancer. I am especially grateful to the multidisciplinary team and all participating centers who contributed to this work.
In this multicenter cohort, sequential carboplatin-paclitaxel followed by dose-dense epirubicin-cyclophosphamide achieved a pathological complete response rate of 60.2%.
A key translational finding was that germline HRD-positive status was associated with higher likelihood of pCR, while baseline tumor phospho-DDR protein expression did not add independent predictive value. Importantly, achieving pCR was strongly associated with better long-term outcomes, with improved disease-free and overall survival after a median follow-up of approximately 5 years.
For me, the main message is simple: platinum-based neoadjuvant chemotherapy remains highly relevant in TNBC. It is part of the backbone of contemporary chemo-immunotherapy, but it also remains important for patients who are not candidates for immunotherapy and for healthcare settings where access to immune checkpoint inhibitors may be limited.
In this context, genomic HRD assessment may help refine our understanding of platinum sensitivity, while static baseline DDR phosphoprotein profiling may not be enough – pointing to the need for more dynamic functional assays.
