“ESMO-ESCAT V2 (2026) is out – How it translates to utilization of broader CGP and impact of MTB (Molecular Tumor Board).
In precision oncology, identifying more genomic alterations is only part of the equation. The real challenge is determining which findings are clinically actionable, which may indicate treatment resistance, and how confidently they should influence clinical decisions.
The updated ESMO ESCAT v2 framework provides a structured approach to classifying the clinical actionability of molecular alterations while introducing a parallel framework for resistance biomarkers.
But how do we translate complex molecular findings into meaningful treatment discussions?
Comprehensive Genomic Profiling (CGP) provides a broader molecular dataset, including actionable alterations, co-occurring findings and potential resistance biomarkers.
ESCAT v2 helps contextualize findings according to the strength and relevance of supporting clinical evidence.
Molecular Tumour Boards (MTBs) bring together clinical, molecular and therapeutic expertise to evaluate these findings alongside tumour biology, patient context, available treatment options and clinical trial opportunities.
The result?
A more structured, evidence-informed discussion that can support treatment prioritization, additional testing and personalized clinical decision-making. The key is not simply to find more alterations. It is to understand which findings should guide treatment, which warrant clinical-trial consideration, and which should not influence treatment selection.
Broader profiling generates the data. Evidence defines its significance. The MTB helps translate it into clinical context.”
