A case of cholangiocarcinoma type IV block from Kazakistan

“October ’23: Young male, early 30s, from Kazakistan with cholangiocarcinoma type IV block. Serum Bilirubin of 12mg/dl. No extrahepatic disease on TP PETCT.

Plan-Three system PTBD stenting followed by intraductal biopsy followed by Radio-frequency ablation. 3 days Post procedure, s.bilirubin came down to 3mg/dl but developed acute pancreatitis (procedural related).

Percutaneous Transgastric catheter was placed to drain the WON. Internalisation of transgastric catheter was done under fluoro-guidance. Patient got discharged.

Repeat PET CT after 3 months shows interval resolution of disease and patent STENTS and no IHBRD.

After 6.5 months patient again visited to hospital with acute cholecystitis. USG and fluoro-guided Percutaneous LAMS placement was done. Percutaneous drainage catheter was placed into GB lumen for 4 weeks as a protocol.

1 month later, tube cholagiogram done to confirm the free flow from gall bladder to duodenum through LAMS. Patient got discharged catheter free with a big smile on his face. Next follow up after 3 months.”
