“Which silicone breast implant should be selected to minimize the risk of silicone leakage? Low cohesive implants are softer than the new generation of highly cohesive implants, but do they leak more silicone over time and does cohesiveness affect the amount of silicone leaked in cases of implant rupture?

Minimizing silicone leakage is important as silicone leakage is a suspected risk factor for various complications, including capsular contracture and lymphadenopathy. Therefore, this is an important question when guiding patients in implant selection prior to a breast reconstruction or breast augmentation.

In this study, we have included data from 493 patients contributing with biopsies from 872 breasts. Our study showed that implants with highly cohesive silicone are superior to low-cohesive implants in preventing silicone leakage, both from intact implants and in case of implant rupture.
