Thomas Stoop: Neoadjuvant therapy versus upfront surgery with left-sided resectable pancreatic cancer
Thomas Stoop, PhD Candidate in Hepato-Pancreato-Biliary (HPB) Surgery at Amsterdam UMC, shared a post on LinkedIn:
“Do you consider a patient with resectable pancreatic body/tail cancer to be a candidate for neoadjuvant therapy instead of upfront surgery?
Published today in Annals of Oncology, our global observational study with 76 centers from Norway, Sweden, the United Kingdom, Germany, Belgium, the Netherlands, France, Italy, Spain, Portugal, Austria, Turkey, Singapore, South Korea, Japan, the United States, and Australia, investigating the value of neoadjuvant therapy versus upfront surgery among 2282 patients with left-sided resectable pancreatic cancer who underwent resection.
Neoadjuvant therapy associated with increased survival compared to upfront surgery
… adjusted 5-yr survival of 47% vs 35%.
The effect of neoadjuvant therapy increases in patients with larger tumors and higher CA19.9 at diagnosis.
Key is Anatomy-Biology-Condition (A-B-C) approach of pancreatic cancer.
Randomized trial needed to give the definite answer.
A wonderful collaboration between Sahlgrenska University Hospital, University of Colorado Anschutz Medical Campus, Amsterdam UMC and 73 other centers from the USA, Asia, Oceania, and Europe team effort.
In particular, I would like to thank shared first authors Elena Rangelova and Tess van Ramshorst for this fantastic team work, Ammar Asrar Javed, Sohei Satoi and Daisuke Hashimoto for facilitating the global coordination, statistician Mahsoem Ali for his crucial role with his expertise together with Ewout Steyerberg, and supervisors Hanneke Wilmink, Mohammad Abu Hilal, Marc Besselink and Marco Del Chiaro.
