Walid Kamoun: Can the Post-Surgical BBB Become an Ally Against Glioblastoma?

“Getting therapies through the BBB has constituted one of the challenges of brain tumor systemic therapy. BBB disruption is heterogeneous leading to heterogenous delivery.

Most patients with glioblastoma undergo resection surgery, often within a short period of diagnosis. What happens in the hours and days right after that operation may matter more than we thought.

In preclinical mouse models, Fernandes et al. mapped windows where the blood-brain barrier is disrupted at the resection margin: immediately after surgery, and again at 48 to 72 hours.

Liposomal nanoparticles given intravenously during those windows accumulated selectively at the margin, with minimal penetration elsewhere in the brain. Timing a single dose of doxorubicin-loaded liposomes to those peaks improved delivery to the brain and inhibited tumor recurrence across two models.

Interesting concept: leveraging a by-product of surgery, the spatial and temporal disruption of the BBB, to target residual disease.
