Olivier Elemento: A Roadmap for AI in U.S. Medicine

Olivier Elemento, Director of Englander Institute for Precision Medicine, shared a post on LinkedIn:

“Leading the Transformation: A Roadmap for AI in U.S. Medicine

Today, my colleagues at the Englander Institute for Precision Medicine and I submitted the attached response to The National Institutes of Health of Health’s call for a national AI strategy. The NIH’s stated goal is to chart a course from today’s analytics to a future of autonomous AI—a fundamental shift for biomedicine. We were pleased to offer a concrete and comprehensive roadmap to help realize this vision.

Our proposal is built on a few core ideas:

  • The global race to lead in biomedical AI is a matter of both national health and economic security. With nations like China making massive state-directed investments and drug development costs averaging over two billion dollars, a decisive U.S. strategy is essential.
  • We outlined a phased roadmap for AI, starting with ‘co-pilot’ agents that assist scientists and evolving towards fully autonomous platforms that can design and execute entire scientific experiments with high-level human oversight. We believe this long-term vision is necessary to unlock truly transformative discoveries.
  • A core principle we advocate for is treating clinical AI tools like therapeutic interventions. This means they must be held to the same high evidentiary standards as new drugs, including prospective Randomized Controlled Trials. This also requires generating new, large-scale datasets with AI as the primary consumer, structured for machines first.
  • We also think it is important that the NIH leads by example. By using AI to streamline its own operations, such as reducing the average R01 grant award time from a baseline of nine months, it can serve as a model for the entire biomedical enterprise.

The rapid advancement of AI demands a bold, commensurate national response. We believe an ambitious, well-resourced plan will catalyze a new golden era of American-led biomedical discovery and fundamentally transform the future of human health.”
