Gustavo Viani: Prophylactic Pelvic RT with SBRT for High-Risk node negative Prostate Cancer

Gustavo Viani, Professor of Radiation Oncology at Ribeirão Preto Medical School, shared a recent article by Vedang Murthy and colleagues on X:

“Breaking: Prophylactic Pelvic RT with SBRT for High-Risk node negative Prostate Cancer.

New results from the SHARP consortium show that 25 Gy /5 fractions achieve 98.2% pelvic control, offering a groundbreaking approach to regional control in high-risk, node-negative prostate cancer.

Methods:

Pooled analysis of 171 people treated across Multi-center data from the U.S., Canada, and India under the SHARP consortium.

Regimen:

  • Prostate SBRT (35–40 Gy/5fr ) + Pelvic RT (25 Gy/5 fraction).
  • Delivered with IGRT (CB CT).

 Key Outcomes:

  • Median FU 51 months.
  • 5-Year BFFS: 86.1%.
  • 5-Year OS: 89.3%
  • Only 1.8% pelvic recurrence all with distant mets.

Clinical implications:

This regimen is effective, safe, and supports the growing adoption of ultra-hypo for high-risk. It is efficiency with strong clinical outcomes.
