Samantha Siegel: Onco-Primary Approach to Symptoms in Survivorship

Samantha Siegel, Director of Cancer Survivorship at The Permanente Medical Group, shared a post on LinkedIn:

“Onco-Primary Approach to Symptoms in Survivorship:

Sharing a slide that I made to explain an onco-pcp’s approach to symptoms in cancer survivors (including those on active treatment and/or after the end of active treatment.)

I conceptualize symptoms into three main categories:

1. IS THIS CANCER RELATED?

A relapse, recurrence, progression or subsequent cancer?

2. IS THIS CANCER TREATMENT RELATED?

Especially in the era of Immune Checkpoint Inhibitors, targeted therapy, antibody drug conjugates and more, we HAVE to consider the mechanism of the therapies and evaluate current symptoms in that context. For some therapies such as ICIs, symptoms can occur even up to a year out from the last infusion, so it may not be on everyone’s radar.

3. IS THIS UNRELATED?

Sometimes a cold is just a cold and it has nothing to do with cancer. However, in a survivor, cancer often remains a consideration even when the condition is not directly related.

I find that verbalizing this framework helps to reduce stress all around and to partner with patients for the best outcome.”

Foundation for Autoimmune Cancer Support shared this post on LinkedIn, adding:

“Great summary – people are surviving with cancer in unprecedented numbers. But life happens: ailments, injuries, illnesses, etc.

How does one create a framework to help our patients advocate for themselves and how do busy physicians think about how to care for patients?

Thank you, Samantha Siegel, MD for putting this out there!

The future of all internal medicine is going to be managing disease along side of cancer!”
